KOSRA has been asked by the Eastern Cape Department of Health to assist with the distribution of the COVID-19 Screening Form.

This will enable the Department to ascertain who needs to be tested. Those who DO need to be tested will be contacted by the Department.

Everyone living in NDLAMBE at the present time needs to complete the form. Complete one form for every person in the household.
